The easiest of all is to pick a stitch/pattern you know and like; make a long and wide scarf (rectangle); then sew or crochet the two short ends together. Edge or fringe is as you like. :)
Instruction video "Spinrite shows you how to make a poncho from a scarf (http://acmoore.com/video.asp?ID=56) "
As for poncho patterns:

One of the easiest ways of making a plain poncho is to make two rectangles in whatever stitch you prefer. If you look at this poncho pattern (http://freecrochetpatterns.lionbrand.com/patterns/ccw-childsPoncho.html)
it shows you how to put it together once you've got the two rectangles. It also gives you 2 sizes of rectangles for two age groups.
I followed the basic sizes in this pattern to make a poncho for my daughter- I used different yarn and a different hook but since you're going from measurements it doesn't matter all that much.
(Just looked at the links the previous poster gave- didn't realise they'd already given you this link oopps.)
