Chapstick Holder?

I saw one of these clipped to the bag belonging to a lady in line at the grocery store a while back. I've been wanting one for ages, but have no idea how to make one! She said that she'd gotten hers as a gift from a friend who crocheted, but didn't know how she'd made it.

Has anyone else seen these? I really want one/want to make one but haven't had any luck finding a pattern.
