Pet Angel

Hope someone can help me. A few weeks ago I saw a link for Pet Angel that was made for someone who had just lost their cat. A friend of mine just lost her dog and i thought that it would be nice to make her one of these. I can't seem to find this link again. Anyone know where I can find it?

I will have to look for this, I would like to make one for my neighbor, who lost her dog about a month ago.
