I have searched to no avail with the exception of one pattern at annies attic I think. I used to have a booklet from Leisure arts I believe but I cannot find it now.:(

Here's a link to free filet alphabets. All you'd have to do is find a border you like: http://crochet.about.com/od/alphabetcharts/Alphabet_Charts.htm

This one in particular lets you type in the name or phrase you want: http://crochet.about.com/gi/dynamic/offsite.htm?zi=1/XJ/Ya&sdn=crochet&cdn=hobbies&tm=129&gps=418_545_1020_556&f=22&su=p445.92.150.ip_&tt=14&bt=1&bts=1&zu=http%3A//www.celticxstitch.ie/

This may be the leaflet you are thinking of, but it's out of print.

Maybe with the name and #, you can find it somewhere?
Good luck.

There's a good one called "Tapered Name Doilies" by Vivian Floy Smith.
It's leaflet # 610 from Heirloom Creations. The copyright is 1998.
The email address on the booklet is: heirloom@tgn.net, but I tried their web address from the booklet, and it's no longer good. I guess it's OK to give the phone # and physical address, since they're printed on the back cover of the booklet, so here goes:

Heirloom Creations, 1107 Elm Street, Sweeny, TX 77480
409-548-3141

I hope some of that information is still current for you!
